⁠3. At what stage of her perimenopause/ menopause journey should a woman start any kind of menopause hormone therapy?

There is a phase before menopause that is as important as menopause itself—the perimenopause! How you show up in your perimenopause will determine the severity of your symptoms during this transition period.

Oestrogen is a highly anti-inflammatory, anti-depression hormone produced by the body for our protection, including our brain protection. Since hormone levels begin fluctuating and changing 7-10 years before menopause during perimenopause, which is a transition stage, we really don’t want to wait for symptoms like brain fog, hot flashes, and fatigue to take over our bodies. 

To understand this better, let’s just look at a few numbers!

  • ⁠ ⁠Studies show that there is a 30% drop in brain energy during perimenopause (Dr. Lisa Mosconi). This cognitive decline doesn’t have to be inevitable—early hormone balancing can help protect mental clarity.
  • ⁠  ⁠A 30% increase in cardiometabolic risks, including insulin resistance, occurs as oestrogen decreases. Therefore, balancing hormones earlier reduces these risks, safeguarding your heart and overall health.
  • ⁠  ⁠Hormone therapy has been shown to decrease the likelihood of diabetes in perimenopausal women. So, why risk developing a condition you could potentially prevent?
  • ⁠  ⁠According to a report published in the British Medical Journal, the drop in oestrogen is directly correlated with an increased risk of dementia. Supporting and restoring oestrogen levels earlier on can slow or prevent cognitive decline, keeping your brain sharp as you age.

The initial 10 years of menopause (which officially starts 12 months without a period) are critical for oestrogen protection. However, according to a Women’s Health Initiative study, oestrogen’s protective benefits significantly decrease after 60.

Richard Morgan Evans

Richard co-founded Sapience with three highly successful business figures in the UK, including former bank and private equity CEOs. This followed previous careers in law, working as an equity analyst for a number of investment banks and financial journalism, which included writing for The Economist, The Times and The Independent.

During his journalist career, he visited the Gulf and Saudi Arabia many times, interviewing senior banking and business figures, such as the head of the Saudi Central Bank and leaders at Saudi Aramco, SABIC, Olayan Group and the Kanoo Group. A number of his features appeared in the Wall Street Journal, Financial Times and The Washington Post.

His PR experience includes being the MD of the Financial Division of a leading UK multidisciplinary PR consultancy before co-founding Sapience. He is often asked to judge PR awards, most recently he was on the judging panel of the Public Relations Consultancy Association’s Financial Services PR awards.

He specialises in advising on strategic media relations, reputational risk, media relations and training, crisis communications, and all forms of corporate transactions. Richard is a law graduate of the University of Cambridge.
