Please meet P.V.,, one of our patients from Saudi Arabia:
Age: 48
Date: May 2026
Diagnosis: Perimenopause
Symptoms: Nervousness, sleep issues, memory problems, difficulty concentrating, discontent, loss of interest in sex, decreased libido, tiredness, breast tenderness, hot flashes, night sweats, weight gain, constant forgetfulness, stress, and brain fog
Treatment: 2-month Bioidentical Hormone Therapy (āBHRTā) protocol
After the initial 2-month Bioidentical Hormone Therapy (āBHRTā) ā during which P.F. was receiving a 2-mg Estradiol dose ā P.Fās serum Estradiol level unexpectedly increased to 2,248 pg/mL.
We instructed the lady to stop all hormone medications for two days before repeating the test literally in 2 days. The repeat Estradiol level dropped substantially ā by 80 folds to 27.6 pg/mL.
This case reminds us that hormone therapy is about optimisation, not maximisation.
The Patientās symptoms had improved considerably after starting BHRT, the patient said that she would have noticed a much greater effect, particularly regarding sexual function and libido, if such a high Estradiol level were truly providing additional benefit.
The objective of BHRT is not to create the highest possible hormone levels, but to restore hormonal balance and improve quality of life, protecting the body from any age-related conditions later in life.
At Medi-Gyn, we focus on the combination of
- symptoms
- how a patient is feeling
- clinical assessment, and
- laboratory values to determine the most appropriate treatment strategy for each woman
Conclusion:
You can just see it for yourselves, that was how rapidly the Estradiol level declined after only two days without applying your Estrogen cream. This demonstrates how quickly hormone levels can fall when treatment is interrupted and highlights the importance of consistent, regular use of BHRT.
The significant drop in Estradiol in a short time ā 2 days ā tells us that we must be consistent with our hormone therapy in order to maintain stable hormone levels and sustain the clinical benefits of treatment.
